In Care of Name:	The responsible party for the gown if the member is under the age of 18.
Role:	The member's role in the wedding party. For example: Maid or Matron of Honor (MOH), Bridesmaid (BM), Junior Bridesmaid (JR), Flower Girl (FG), Mother of the Bride or Groom (MOM)
Manufacturer:	The maker of the gown you would like. For example: Jordan, Jasmine, Forever Yours, Alexia, etc. Please visit our Links and Manufacturer's pages for a complete list of designers.
Style Number:	The internet or actual style number of the gown(s) you would like.
Color:	The gown and/or trim color you would like.
Telephone:	List the contact information for the responsible party for each gown. We will contact you when your registration form has been received to verify all the information as well as pricing and any discounts.
